*Live Demonstration

I understand that the brow lamination and dye treatment will be demonstrated live in front of an audience (whether in-person or online). I agree to participate in this live demonstration.

*Availability & Location

I confirm that I am available on Sunday, 13th April 2025, from 12:45 PM to 3:00 PM, and agree to attend during this entire time for the demonstration.I understand that the treatment will take place at:

*Time Commitment

I understand that the brow lamination and dye treatment may take up to 2 hours, and I understand that I will need to remain still for the duration of the procedure.

*Eligibility

I confirm that I have not had a brow lamination treatment in the past 8 weeks, and I understand that this is a requirement to participate as a model.

*Photo and Video Consent

I understand that photos and videos will be taken during the session for promotional, educational, and marketing purposes. I give permission for these images and videos to be used on social media, websites, and other marketing platforms.

*Health and Safety

I confirm that I do not have any known allergies or conditions that may affect the treatment, such as:

• Sensitive skin

• Any broken skin in the treatment area

• No sunburn in the area

• No prior reactions to the products used in brow treatments.

I understand that it is my responsibility to inform the technician of any allergies or sensitivities prior to the treatment.

*Treatment Expectations

I acknowledge that individual results may vary, and the final outcome of the brow lamination and dye treatment may differ from my expectations based on my natural brow shape, color, and texture
