• Have any damaged/broken skin.

• Sunburn in the area.

• Be on any acne medication.

• Used retinol or had any form of peel done in the past 7 days.

I understand that if I am a model for the following I must not of had any prevoius brow services in the past 4 weeks.

• Brow shape only (wax)

• Brow tint & shape & wax

• Brow dye & shape & wax

I understand that if I am a model for a brow lamination I must not of had a brow lamination service for a minimum of 8 weeks prior.*

I understand that by submitting this form, I confirm that I have not experienced any reactions or allergies from previous brow services.*
